- Provider Agreement During my many years of providing day care, I have developed and refined a Child Care Contract that serves as an agreement between parents and myself, as the day care provider. This document establishes the terms of the service provided and includes, but is not limited to, the following items: Trial period Hours of Operation Fees Pickup and Dropoff Illness Nap Time Toys and Food Discipline Meals Supplies Clothing Transportation Field Trips Emergency Care Sun Protection Termination of Service |
